Inguinal Hernia in Men: Causes, Symptoms and Treatment

What Is an Inguinal Hernia and Why Does It Mainly Affect Men?

An inguinal hernia in men is one of the most common urological and surgical conditions seen in male patients. It occurs when internal tissue, usually a portion of the intestine or abdominal fat, pushes through a weak spot in the groin muscle wall. The result is a visible or palpable bulge that may or may not cause discomfort, but always requires medical evaluation.

Men are significantly more prone to this condition than women due to anatomy. During fetal development, the male testicles descend from the abdomen into the scrotum through the inguinal canal, leaving a naturally weaker area in the abdominal wall. Over time, physical strain, excess weight, or simple aging can widen that weakness and allow a hernia to develop.

Types of Inguinal Hernia

Direct Inguinal Hernia

This type develops gradually, most commonly in middle-aged or older men. It occurs when abdominal tissue pushes directly through a weakened spot in the muscle wall without following the path of the inguinal canal. It is strongly associated with repeated physical strain and age-related muscle deterioration.

Indirect Inguinal Hernia

The most common variant, it can occur at any age including in young men. It follows the natural path of the inguinal canal and, in more advanced cases, can extend into the scrotum, becoming what is known as a scrotal hernia. This type is related to a congenital weakness in the canal.

Symptoms of Inguinal Hernia in Men

Symptoms vary depending on the size of the hernia and whether complications have developed. The most frequent include:

  • A visible bulge in the groin, especially when standing, coughing, or straining
  • Discomfort or pain in the groin area that worsens when bending over, coughing, or lifting
  • A feeling of heaviness or pressure in the affected area
  • Pain or swelling around the testicles when displaced tissue descends into the scrotum
  • In some cases, the bulge disappears when lying down and reappears when standing

It is worth noting that some hernias are asymptomatic in early stages. However, ignoring the first signs can lead to serious complications. If you experience sudden severe groin pain, nausea, or an inability to push the bulge back in, seek immediate medical attention: this may indicate a trapped or strangulated hernia, which is a surgical emergency.

Causes and risk factors

There is not always a single identifiable cause, but the most common factors that contribute to inguinal hernia in men include:

  • Congenital weakness of the inguinal canal
  • Overweight or obesity, which increases intra-abdominal pressure
  • Chronic physical strain: weightlifting, heavy manual labor
  • Chronic cough related to smoking or respiratory conditions
  • Persistent constipation causing repeated straining
  • Previous abdominal or pelvic surgery
  • Family history of hernia

Age also plays an important role. Although indirect inguinal hernias can appear in young men, the progressive loss of muscle mass over the years increases the risk. A groin lump does not always mean hernia: conditions such as varicocele can produce similar symptoms in the scrotal area. If you are already experiencing discomfort in that region, it is worth reviewing the Varicocele: 5 Warning Signs You Should Not Ignore to rule out other causes.

Diagnosis: How is Detected the Inguinal Hernia

Diagnosis begins with a physical examination by the specialist. The physician palpates the groin area while the patient coughs or strains, which allows identification of the bulge and assessment of its characteristics.

When the diagnosis is unclear or a scrotal hernia is suspected, imaging studies such as ultrasound or CT scan may be ordered. These tests help determine the size, contents, and type of hernia, information that is essential for planning the appropriate treatment.

Treatment: When Is Surgery Necessary?

An inguinal hernia does not resolve on its own. It tends to grow over time and increase the risk of complications. The only definitive treatment is surgical, though the timing depends on symptoms and the patient's overall health.

  1. Herniorrhaphy (Repair Without Mesh): This technique involves closing the muscle defect with sutures. It is used primarily for small hernias or in younger patients. Its main advantage is that it does not involve any prosthetic material.
  2. Hernioplasty (Mesh Repair): Currently the most widely used technique. A surgical mesh is placed over the weakened area to reinforce it, significantly reducing the risk of recurrence. It can be performed through open surgery or laparoscopy. The laparoscopic approach allows faster recovery and less postoperative discomfort.

Small hernias without symptoms may be monitored over time under medical supervision. However, in active men or those with progressive symptoms, surgical repair is generally recommended without delay. Just as with varicocele, where the condition may worsen over time without treatment, delaying intervention in a diagnosed hernia increases the risk of complications.

When should you see a specialist

Schedule a consultation with a urologist or specialized surgeon if:

  • You notice a bulge in the groin that appears and disappears with exertion
  • You feel discomfort or pressure in the groin or scrotal area
  • The bulge becomes hard, painful, or cannot be pushed back manually
  • You experience nausea, vomiting, or fever along with groin pain
  • You have a family history of hernia and engage in intense physical activity

At Endolapv, Dr. Salvador Bricio provides complete urological evaluation for the diagnosis and treatment of inguinal hernia in Puerto Vallarta. The consultation includes a detailed physical examination and, when required, coordination of imaging studies to confirm the diagnosis before defining a treatment plan.

Conclusion

Inguinal hernia in men is a treatable condition with a high success rate when addressed in a timely manner. Recognizing the symptoms, understanding the causes, and acting before complications arise are the key steps to a full recovery. If you have questions or identify any of the signs described in this article, schedule a consultation with Dr. Bricio at Endolapv in Puerto Vallarta. Early care is always the best decision.
